Single Hazard Risk Assessment and Action Plan Standard Format

Simply-docs has introduced a new style of Risk Assessment Form and Action Plan to replace this format.
This form provides in a single document the 5 Steps to Risk Assessment recommended by the HSE:
1. Look for and identify the hazards;
2. Decide who might be harmed;
3. Evaluate the level of risk(s) arising from the hazards and decide whether existing precautions are adequate or more should be done;
4. Record your findings;
5. Review your assessment from time to time and revise it if necessary.
This form contains the following sections:-
1. General Information
2. Description/Identification of Hazard
3. Who is at risk?
4. Level of and associated risk
5. Precautions and Controls - improvements and new recommedations
6. Action and further action to be taken, by whom and target dates
7. Review timetable
This style of form is suitable for small office based businesses where the nature and size of the operation(s) limit the likelihood of hazards and extent and severity of the risk. Risk assessment must be "suitable and sufficient" to provide for health and safety risks to employees whilst at work and to non-employees who may be exposed due to the way in which any aspect of the business is conducted.
The risk assessment must be appropriate to the nature of work, and be understandable and repeatable.
The Multiple Risk Assessment and Action Plan provides a more compact style of risk assessment.
Businesses where there are substantial hazards or potential hazards, and the risk(s) could be severe and extensive it is recommended that the risk assessment cycle should be managed by the application of the relevant Checklist(s), Single Hazard Risk Assessment Form and Risk Assessment Action Plan.
